To make an appointment with one of our Urologists, we first require a referral.
Please use this form to upload your referral to our office. If you have had any pathology or radiology investigations completed (for example PSA blood tests, urine tests, ultrasound, CT or MRI), please let us know where these were performed (e.g. Clinpath, Benson’s Radiology) in the “Additional information” box.
Once we have received your referral, our administration team will contact you within a few business days to arrange an appointment.
